A 22-year-old woman was admitted with cough, hemoptysis and decreased strength in the right hemibody that evolved to quadriplegia, bilateral Babinski’s sign and T4 sensory level. Imaging revealed lesions compatible with pulmonary tuberculosis; sputum smear was positive and MRI showed lesions compatible with longitudinally extensive transverse myelitis. She received methylprednisolone pulses and antituberculosis treatment, slowly improving.
